Between Transient Heartbeats: The Emotional Trials and Triumphs of ICU Clinical Instructors
Abstract
Clinical instructors in the intensive care unit (ICU) navigate a complex, high-stakes environment where effective teaching demands profound resilience, rapid decision making, and robust collaboration to guide students while safeguarding patient safety. The primary objective of this study was to explore and describe the lived experiences of clinical instructors assigned to critical care units. Utilizing a qualitative research design anchored in Husserlian descriptive phenomenology, the study employed purposive sampling to select eleven participants, with rigorous ethical approval secured from the Ethics Review Board prior to data collection. Data analysis revealed four emergent themes characterizing the instructors' experiences: (1) From Fear to Courage: Emotional Transformation of Clinical Instructors in the ICU, highlighting the trajectory of overcoming initial apprehension to build enduring confidence; (2) Forged in Crisis: The Mastery of Skills Amidst Critical Moments, detailing the acquisition of clinical competence and real-time decision-making capabilities; (3) Teaching on the Edge of Life: The Burden and Blessing of Guiding Amidst Crisis, reflecting the profound emotional toll inherent in ICU care; and (4) No One Stands Alone: Strength Drawn from the ICU Team, emphasizing the critical role of collaborative care and mutual support in high-pressure environments. The findings underscore the imperative for healthcare institutions and academic organizations to establish comprehensive support systems addressing both the personal and professional needs of ICU instructors. Specifically, the implementation of formal mentoring programs, structured peer support groups, and accessible psychological services is recommended to mitigate emotional distress. Furthermore, establishing institutional recognition programs is vital to sustain motivation and validate the indispensable contribution of ICU clinical instructors to both patient care outcomes and the advancement of nursing education.
